By scheduling this appointment via “virtual” or telehealth you are confirming that that you are an adult age 18-64 years and you are the person scheduling your appointment and consenting for this service voluntarily. You are also acknowledging that you have been made aware of the following, including our cancellation policy, and wish to proceed with the visit given these terms.

If you do not wish to have a telehealth visit but instead want to be seen at your home, office or hotel in our mobile service area please visit our website at www.ForbesVIPHealth.com for information on that type of visit.

All mobile visits for in person care at your home, office or hotel in our mobile service area will require payment of a separate, additional fee at time of scheduling in order for the provider to come to your location.

Once you have scheduled your appointment type, you will be redirected to the payment page to enter your payment information for the visit.

All telehealth visits are conducted via a private, HIPAA compliant platform and you will have access to the information resulting from the telehealth service as required by law.

All parties present at the receiving and originating site for the telehealth encounter will be informed of who is present and have the right to exclude anyone from either site. You have the right to object to videotaping or other recording of the telehealth visit.
